mas o Usuário só pode escolher uma opção por vez depois ele deve retornar a tela inicial ou encerrar a operação

no primeiro código o usuário tem que escolher só uma opção(marcar só uma opção) e em seguida ser redirecionado para outra pagina. eu estava pensando em usar a função radio mas não tenho certeza se vai funcionar eu tenho que puxar os dados para o php do primeiro programa para exibir o nome de usuario se o resultado for negativo ele volta para a pagina inicial.
